Default DPS-9000 connection problem

The power pack shows fully charged when plugged in to the wall and the interface cable light is green but when I switch over to use the camera (Dimage 7 Hi) and check the power charge nothing lights up. I have switched out cables as I have duplicates and still not getting power to the camera via the battery pack. It is not a problem with the camera because I have installed the AA batteries in it and it works just fine.

My questions are:
1) how do you tell when the pack is completely dead and needs replacing? It's like a false positive when you check the lights when plugged in to the wall, all lights are green and the interface light is green.

2) can just the battery be purchased since I already have two complete sets of cables and don't need more.

3) Is it best to unplug everything when I don't use the camera that often or is it okay to leave the pack plugged in to the camera and ac so it is always ready to go? ( I have always left both plugged in for ready to go.)

I love using this with my camera so any help would be appreciated.
