Default 1280 Ink Puddle - Blown Cart. or Printer?

I ran a 4x5 test print on Photo Quality Injet paper (I had not used the printer in 3 weeks) and was amazed when the print emerged with a solid puddle of ink in the middle of the paper, bigger than the 4x5 image size, and trying to run off the sides. I contained the puddle, removed the paper, and removed the cartridges. They were not leaking when I removed them. Looking into the printer, the foam pad is soaked in ink to the left of the 8" paper path, and to the right, both the 1/2" or so before the pad ends, the over the plastic and on the pad where the print head parks.

I cleaned up the surfaces, replaced the head, and tried a print test. This produced 2 solid hevily soaked horizontal bands 3/8"+ wide, one solid blank, and one with a mix of colors. The bands are separated by 5/16". The black band is edge to edge and the colored band is over the right edge but 1 1/4" short of the left edge. As before, the foam pad is soaked on either side of the paper path.

Does anyone have any thoughts or experience to share? Blown cartridges or blown print head? Something else?

By the way, the cartidges all show 3/4 full, and were freshly opened for my last successful run of prints, 3 weeks or so ago. At that time I had to run the clean print heads operation 12 times or so to get going, but after that I did a fair amount of printing with no hitches.
