Can you provide an example?

One thing that can contribute is if you are very close to the subject, you will get flash burn. Also, if you force the flash on when outside, the synch speed of the camera (1/200) is too slow for otherwise sunny conditions which will also result in overexposure. So, post an example along with the EXIF data.
