Hill and Usher based in Arizona offer a photographers insurance that provides liability insurance of $1 million, covers errors and omissions and equipment through the Hartford Insurance company for around $500-$600/year. Pretty reasonable in my mind for a working full time or part time pro.

If your just looking to cover your equipment, you can purchase a rider through your homeowners insurance for even less. Be careful though, too many claims could jeopardize your coverage, and if your equipment is being used to make any money, it is technically not covered.
