This happend to me twice while photographing lightning. I just attributed it to the emf pulses from the lighting scewing with my camera. They were at least 3 second exposures.

It also happend once while using the remote with a 1 second exposure.

So just to make sure that I got this right:

Twice this happened to you when using long exposures WITHOUT the remote

Once this happened to you when using a 1 second exposure WITH the remote

Is that right? :O
