No that's what's confusing in Photoshop it looks correct.

I just downloaded Irfanview and will give it a try also.

the extra 1/8th inch may have come from the borderless setting. I know you printed in on 8.5 x 11 but you probably still had the borderless box checked.

The computer has to "bleed" the image over the edge to get a borderless print. 1/8 inch is a common bleed over because no paper is perfectly square much less perfectly 4x6

the other stuff I have no idea about. we have a cheap greeting card making software "printmaster 15" and it always screws up the orientation.

make sure you have downloaded the latest drivers and software patches for your printer.
