To leave on or not to leave on that is the question.

Do you Pentax Dust removal owners leave it on to cycle through every time you turn the camera on, or leave it on manual mode to use when you desire?

Same here. I leave it turned on to run every time I turn the camera on. I figure, "why not? It can't hurt."

I didn't use it much most of the time when I had the K10. However, I've left it on with the K20 and it hasn't hurt so far. Not sure it makes all that much difference though, I usually am careful how I change lenses.

I only use it when I notice a spot on my images.

With two bodies, an *istDS as well as the K10D, I tend to change cameras not lenses out in the field so I seem to be geting less dust on the sensors.
