CT - I used Aperture with the Topaz suite.

1. Selective sharpening of the candles & crown (not the face) using Aperture's built in brush (this is a favourite tool of mine).
2. Topaz InFocus - just a touch as too much had an horrendous effect !
3. Topaz Denoise - around 1.0
4. Selective sharpening of the area around the eyes and mouth - nowhere else to preserve the smooth look to the skin (though at 6 yrs old it will be smooth anyway) !

To add to frogfish's posting: the image responds quite fine to some basic deconvolution, too.
