When shooting with ISO Hi200 and RAW+JEPG the JPEG comes out exposed properly, i.e., the histogram on the camera and the histogram in PhotoShop are pretty much the same. However the histogram for the RAW version is about one stop underexposed.

I have not seen that happen with other ISO settings, but is it unique to my KM 5D or does it happen with all of them?

The Hi200 (and Lo80) setting is mostly handled in the post processing the camera does on the image before saving the JPEG file. Hi200 & Lo80 will only give you the details in High Key and Low Key situations if you're saving a JPEG. Otherwise you have to do it in post processing using Camera Raw, Adobe Lightroom or your favorite RAW processing program.
